The Harbor Corridor Best Babies Collaborative will be creating a system of care that would coordinate outreach, referral to services needed, case management, tracking and follow-up, health education, provision of prenatal and interconception care, mental health and social support.
- Outreach workers, nutrition and referral assistants and intake workers identify patients and assess for services needed and refer to appropriate partners and services
- Case Managers provide intake and develop management plan
- Case Managers make appropriate referrals to services needed
- Referral to and monitoring of health education, interconception care & social support services
- Provision of mental health and social support services

- Youtube video on St. John's Healthy Homes/Healthy Kids program
- Direct coordination between clinics and hospitals
- Case management: cross reference, case conferences and direct coordination between agencies

Harbor Corridor Best Babies Collaborative Service Area & Target Population:
The Harbor Corridor Best Babies priority target areas include 90001, 90003, 90006, 90007, 90011, 90015, 90037, 90043, 90044, 90047, 90057, 90062. Our targeted population will include but not be limited to women living below 300% FPL, teens, African American Women and women with previous low birth weight outcomes, women needing interconception care, uninsured, substance abusers and pregnant women with chronic illness.
